Wholesale Account Request Form

Interested retailers will need to fax us their Tax ID# (or resale certificate) along with proof of business with a copy of your business license and any *one* of the following items: a) a photo of storefront, b) credit references sheet, c) copy of lease (first page), d) a business card, e) a copy of store newsletter/class schedule, or f) store website.  We will be back to you with a password and account number within three business hours.
